Security, access-control, and PHI-handling changes are not itemised publicly. For a clinical system, a public list of which control changed and when is an attack map rather than a release note. Customers receive the full detail through their engagement, and our security posture and disclosure process are documented in the Trust Center.

    A records document library, and a defensible answer to a subpoena

    Every document held against a case now lives in one library, searchable and filterable, at case level and at patient level. From it a production packet can be assembled for a subpoena or a records request: the responsive documents, an index, and a written basis for anything withheld.

    Packets state what was superseded, duplicated or re-numbered

    Real records arrive with the same report filed twice, a corrected version alongside the original, and page numbering that restarts partway through. A packet now says so on its face rather than presenting the result as though it were a clean sequence, because the discrepancy is what opposing counsel asks about.

    Each document says whether it has already been disclosed

    A document shows whether it has previously gone out, and in which production. Deciding what to send no longer depends on someone remembering what was sent last time.

    A production can be voided without losing the record of it

    A production issued in error can be voided, and its downloadable artifact can expire, while the record that it was issued — and to whom, and when — remains. Withdrawing a disclosure and erasing the evidence of it are different things.

    Phone and tablet controls meet the 44px touch floor

    More than 120 controls across the console were below the 44px minimum touch target on a phone, including the case selector on every route. They now meet it. Clinical work does happen on a phone between rooms, and a control that is hard to hit accurately is a control that gets hit wrongly.

    The BAA exit right has a console surface

    A practice ending its agreement can now exercise its exit right from the console: export the record, request destruction, and receive the destruction certificate. The system refuses a destruction it cannot actually complete rather than reporting one it did not.

    Recurring courses of visits

    A course of treatment can be booked as a recurring series from the schedule instead of one appointment at a time.

    IMR determination letters are uploaded, not retyped

    An independent-medical-review determination can be uploaded as the letter itself rather than pasted in as text, so the source document stays part of the record.

    QME evaluations can move backwards for rework

    An evaluation can be moved between stages in either direction, with a reason recorded when it goes back. Work that needs redoing no longer has to be worked around.

    Objected disclosures link to what replaced them

    Where a disclosure is objected to and re-issued, the two are linked and the Form 109 filing is generated, so the sequence reads as one exchange rather than two unrelated events.

    Records that could be created can now be corrected

    Three record types could be entered but never amended, which meant a typo became permanent. They can now be corrected, with the change tracked.

    Multi-clinic groups

    Practices operating more than one clinic can be administered as a group. Access is granted explicitly per clinic and denied by default, and a physician holding grants across several clinics can switch between them — with the grant re-checked on every request rather than once at sign-in.

    Register exports

    Any register can be exported to CSV or XLSX. Exports are generated on the server and streamed, so a large register no longer depends on what the browser can hold.

    §4610 deadlines run on the clinic’s local clock

    Utilization-review deadlines were computed against the UTC calendar day, which could place a request submitted late in a Pacific afternoon a full business day early. Deadlines are now computed on the clinic’s wall clock, and authorizations already on record were recalculated. A correction only ever moves a deadline later, never earlier.

    Interpreter cancellation billing

    A cancellation with no recorded notice period no longer bills the two-hour interpreter minimum. An explicitly recorded zero-notice cancellation still does — an entered zero is a real figure, not a missing one.

    Fee-schedule gaps report as undeterminable

    Where a fee schedule is missing, an explanation-of-review underpayment is reported as undeterminable rather than as a finding, and no zero-value dispute letter is generated.

    Work in progress is protected

    Closing a tab with unsaved clinical work now warns first. Thirteen states that previously offered only “refresh the page” now offer a real retry.
